This form is to commit to monthly private lessons. please reads the following T&Cs and sign that you agree to the following

• I understand private lessons are monthly

• All private lessons must be paid for in advance at the start of each month.

• per 15 min private is $16

• No refunds or credits if class is missed for any reason

• I understand payment must be paid via the invoicde sent or in person at the studio.

• lesson cannot be past on to another student or sibling if unable to attend

• I understand i must give 1 month notice if i wish to opt out of private lessons.
